Do not feel you need to give anything. Some people give more so that others who cannot give can still have a Sozo. We want that freedom for you.

I understand that team members from Living Fountain Ministries, Inc., have voluntarily agreed to pray for me (using various inner healing methods and when appropriate pastoral counseling). I understand that this session is not a professional counseling meeting and that none of the team members are licensed counselors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more spiritual freedom in my life. I understand that Living Fountain Ministries, Inc., is a non-profit Arizona corporation that makes no charge for its services.
I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to
accept or reject any of the advice or help that I might receive from the team members of this ministry.
I understand that team members offer biblically-based spiritual services to anyone who desires them regardless of ability to pay (but we hope you will see that anything you give will be an investment into your spiritual growth). Although there is no charge for our services, all efforts to build this ministry and to support and train our team members are paid directly from the donations of those receiving these services and friends of the ministry. Because the IRS recognizes that you receive something for your contribution, and because a Sozo session is nationally valued at $75, any contribution under $75 is not eligible for a tax deductible receipt. A Sozo appointment is offered free of charge, and we simply ask that you contribute the amount you are led in your heart to give.
I understand that if I receive ministry from Living Fountain Ministries, Inc., the team is committed to respect the disclosed information, but not to complete confidentiality. I understand that the information, as needed, may be shared with other leaders of Living Fountain Ministries, Inc., so as to further my total healing process.
I understand that in accordance with Arizona law, Living Fountain Ministries, Inc., is required to report two issues: 1. Any intent of a person to take harmful, dangerous, or criminal action against another person or against him/herself, and 2. Any act of child or elderly abuse or neglect.
l agree to hold Living Fountain Ministries and its team members free from any and all liability, loss and damage of any kind that may arise as a result of assistance which I will receive from my voluntary participation in this ministry, or from my involvement with Living Fountain Ministries, Inc.
